First time applying for a passport?

We can help. We have participating libraries to help you process your first time applications (DS-11 form only).


We cannot help with renewals. You will need to visit the State Department’s website for help with the DS-82 renewal form and process.

Prepare for your visit

You will just need five things to make processing your request smooth and worry free:

  1. Download and complete the form DS-11, but DO NOT SIGN IT. Do not use correctional fluid or tape to correct any errors made on the application.
  2. Certified birth certificate or naturalization certificate or previously issued passport for those who cannot use the standard DS-82 renewal form
  3. Valid ID: Hawaii driver’s license, Hawaii state I.D., military I.D. or valid U.S. passport (all other forms of I.D., including out-of-state driver license, may need additional forms of I.D.)
  4. One (1) passport photo
  5. Two personal checks or money orders (Sorry we cannot accept cash or debit/credit card payments)
    1. One check is payable to the Director of Finance – State of Hawaii for execution fee.
    2. The second check is payable to the U.S. State Department for the passport application fee.

Special Instructions for Children Under the Age of 16

Children must follow the above requirements along with the following:

  1. Both parents/legal guardians MUST be present. If a parent/legal guardian cannot appear in person, a notarized Statement of Consent form must be presented with a photocopy of the front and back of the absent parent’s ID.
  2. Evidence of parental relationship must be provided. If a child has only one parent, evidence of sole legal custody or death certificate of the non-applying parent must be presented. This is required even for the renewal applications for children.

Minors Ages 16 and 17

Applicant ages 16 and 17 can appear with just one parent.

A separate check or money order is required for the execution fee and the passport book/card application. The current fees are as follows:

Passport Card Fees

The U.S. Passport Card may only be used for re-entry into the United States at land borders and sea ports-of-entry when traveling from Canada, Mexico, the Caribbean and Bermuda. Passport cards may NOT be used for air travel.

Applicants for both the passport book and the card will only have to pay one execution fee.

Processing time

The normal processing time for a passport is four (4) to six (6) weeks. If you choose to pay the expedite fee, the passport will be sent to you within two (2) to three (3) weeks.
