The Hawaii State Public Library System offers passport services at the participating libraries. We only accept DS-11 U.S. Passport Applications for first-time applicants, minor renewals and qualifying adult renewals.
You should use Form DS-11 if ANY of the following is true:
If you are renewing a ten-year term adult passport issued at age 16 or older that expired less than 5 years ago and is in your possession, go to the DS-82 Adult Renewal Application Guide.
Eligible for Form DS-82 but prefer renewing in person? You can do so by filling out Form DS-11 instead of DS-82. However, there is an extra $35.00 execution fee in addition to a renewal fee.
Traveling within 14 days?
If you are traveling within 14 days (or within 4 weeks if the destination country requires a visa) do not make an appointment at our libraries. Go to the Online Passport Appointment System or call the National Passport Information Center (NPIC) at 1-877-487-2778 (1-888-874-7793 TDD/TTY) to make an appointment at the Honolulu Passport Agency for rush passport service. Proof of travel within 14 days is required.

3) Proof of Citizenship
Applicant must provide ONE of the following:
A valid passport must be submitted. If a valid passport is lost or stolen, Form DS-64 (Reporting Your Passport Lost or Stolen) is also required in addition to Form DS-11.
4) Photo ID & Photocopy
5) One Passport Photo
6) Two Separate Fees by Check
Must be two separate CHECKS OR MONEY ORDERS for the following passport fee(s) and execution fee. NO CASH, CREDIT CARD OR STARTER CHECKS ACCEPTED.

The passport card cannot be used for international air travel. It can be used for re-entry into the U.S. at land borders and sea ports-of-entry from Canada, Mexico, the Caribbean and Bermuda.
Period of validity: 5 years for minors under age 16 & 10 years for ages 16 and up.

2) One Passport Photo – Staple a new passport photo to the application, using 4 staples as instructed by the text in the photo box.
3) Most Recent U.S. Passport – Include your most recent passport that is soon to expire or has expired less than 5 years.
4) Name Change Document – If applicable, include a certified name change document, such as a marriage certificate, divorce decree, or court-order name change document.
5) Fee by Check or Money Order – Include the following renewal fee(s) by check or money order payable to U.S. Department of State and write your full name and birth date on the memo line:
