REPLACING PASSPORTS LOST IN 2023 HAWAII WILDFIRES
Have you lost your passport due to the 2023 Hawaii Wildfires? You can replace your lost passport free of charge through the Disaster Recovery Reform Act of 2018:
- Fill out the Replacement Passport Form (DS-5504) and include a recent passport photo.
- Complete the Lost/Stolen Passport Form (DS-64). Make sure you mention the name of the disaster (“Hawaii Wildfires”), provide the address where the loss occurred, and specify the approximate date of the loss and certify that the replacement passport fees won’t be covered by other sources like a homeowner’s insurance policy.
- If you require expedited service, you can include an optional fee of $60.
If you need an urgent replacement, you can visit the Honolulu Passport Agency in person to fast-track your passport replacement process.

DS-11 PASSPORT APPLICATION GUIDE
Who Should Use Form DS-11?
You should use Form DS-11 if ANY of the following is true:
If you are renewing a ten-year term adult passport issued at age 16 or older that expired less than 5 years ago and is in your possession, go to the DS-82 Adult Renewal Application Guide.
Eligible for Form DS-82 but prefer renewing in person? You can do so by filling out Form DS-11 instead of DS-82. However, there is an extra $35.00 execution fee in addition to a renewal fee.
Traveling within 14 days?
If you are traveling within 14 days (or within 4 weeks if the destination country requires a visa) do not make an appointment at our libraries. Go to the Online Passport Appointment System or call the National Passport Information Center (NPIC) at 1-877-487-2778 (1-888-874-7793 TDD/TTY) to make an appointment at the Honolulu Passport Agency for rush passport service. Proof of travel within 14 days is required.

DS-11 PASSPORT APPLICATION CHECKLIST
Remember to bring the following:
Before your appointment, print the DS-11 Passport Application Checklist (Printing Version). A version of DS-11 Passport Application Checklist (ADA Accessible) is available.
1) Applicants
- Applicants must be present.
- Ages 16 & 17: Must be with one parent/legal guardian with a valid ID. A minor ages 16 & 17 can apply alone if you have a valid primary ID, PLUS the following:
- A notarized Form DS-3053, Statement of Consent or a signed note from a parent/legal guardian, and a photocopy of the front and back of the parent’s ID OR
- A parent’s check payable to: U.S. DEPARTMENT OF STATE
- Ages 15 & Under: Must be with both parents/legal guardians with valid IDs. If only one parent is present, bring ONE of the following:
- Birth Certificate listing only one parent
- Certified Sole Legal Custody Order
- Absent parent’s notarized Form DS-3053, Statement of Consent and a photocopy of the front and back of the ID
- Form DS-5525, Statement of Special Family Circumstances
- Death Certificate of the other parent
2) Form DS-11 (Application for a U.S. Passport)
Fill out Form DS-11 with black ink. DO NOT SIGN!
If you want to fill out an online form using a Form Filler or to print a PDF and fill out by hand, go to the official Passport Forms website. Print single-sided. If you make an error, complete a new form.
3) Proof of Citizenship
Applicant must provide ONE of the following:
- State-Issued Birth Certificate, Consular Report of Birth Abroad (CRBA), Naturalization Certificate or Certificate of Citizenship PLUS a PHOTOCOPY
- Or a previously issued, undamaged U.S. Passport. Ages 15 & Under: Prior Passport, PLUS legal parental relationship document, i.e., Birth Certificate, Certified Guardianship Court Order.
A valid passport must be submitted. If a valid passport is lost or stolen, Form DS-64 (Reporting Your Passport Lost or Stolen) is also required in addition to Form DS-11.
4) Photo ID & Photocopy
- Valid Government-Issued Photo ID, e.g., Hawaii Driver License, Hawaii State ID, U.S. Military ID, OR Foreign Passport. Out-of-state ID requires an additional ID.
- PLUS, a photocopy of the front and back of the IDs. All photocopies must be single sided on 8.5″x11″ white paper.
- Ages 16 & 17: One parent’s ID. PLUS, applicant’s ID if available.
- Ages 15 & Under: Both parents’ IDs.
5) One Passport Photo
- One 2″x2″ Color Photo
- White Background
- No Eye Glasses, Uniform or Headgear
6) Two Separate Fees by Check
Must be two separate CHECKS OR MONEY ORDERS for the following passport fee(s) and execution fee. NO CASH, CREDIT CARD OR STARTER CHECKS ACCEPTED.

The passport card cannot be used for international air travel. It can be used for re-entry into the U.S. at land borders and sea ports-of-entry from Canada, Mexico, the Caribbean and Bermuda.
Period of validity: 5 years for minors under age 16 & 10 years for ages 16 and up.

Born Abroad on or after February 28, 1983 & Derived U.S. Citizenship through Parent(s)’ Naturalization?
Bring ALL of the following if you don’t have a certificate of citizenship:
- Parent’s Naturalization Certificate, PLUS a photocopy. Both parent’s naturalization certificates if you are born before February 28, 1983. One parent’s naturalization certificate if the parent was naturalized before January 13, 1941
- Child’s Certified Foreign Birth Certificate. If not in English, also a notarized translation
- Child’s Permanent Resident Card or I-551 Stamp
- Parents’ Certified Marriage Certificate. If not in English, also a notarized translation
- Copies of 1040 tax return for the years from the child’s arrival in the U.S. until the parent’s naturalization that show the child as a dependent of the citizen parent
- Proof of legal, physical custody such as parent’s divorce or custody decree if applicable.
Born Abroad to U.S. Citizen Parent(s)?
Bring ALL of the following if you don’t have a Consular Report of Birth Abroad (CRBA) or Certificate of Citizenship:
- Your Certified Foreign Birth Certificate
- Parents’ Citizenship Documents
- Parents’ Certified Marriage Certificate
- Notarized Form DS-5507, Affidavit of Physical Presence or Residence, Parentage, and Support signed by the U. S. citizen parent(s).
Born & Adopted in the United States?
Bring ONE of the following as evidence of citizenship:
- Certified copy of the original birth certificate, PLUS a certified copy of the adoption decree showing the name change OR
- A certified copy of the birth certificate in the adoptive name. If a delayed birth certificate, not filed within one year of the date of birth, also bring supporting documents such as an adoption decree.
Born Abroad & Adopted by U.S. Citizen Parent(s)?
Bring ALL of the following if you don’t have a Citizenship Certificate:
- A final adoption decree, PLUS an official translation if not in English
- Adoptee’s Permanent Resident Card or I-551 Stamp
- Parent’s citizenship document (not required but helpful)

You can use Form DS-82 only if your most recent U.S. passport satisfies ALL of the following criteria:
How to Complete a DS-82 Adult Renewal Application
1) Complete/print Application – Fill out and sign Form DS-82 with black ink. If you want to fill out an online form using a Form Filler or to print a PDF and fill out by hand, go to the Passport Forms website. Print single-sided. If you make an error, complete a new form.
2) One Passport Photo – Staple a new passport photo to the application, using 4 staples as instructed by the text in the photo box.
3) Most Recent U.S. Passport – Include your most recent passport that is soon to expire or has expired less than 5 years.
4) Name Change Document – If applicable, include a certified name change document, such as a marriage certificate, divorce decree, or court-order name change document.
5) Fee by Check or Money Order – Include the following renewal fee(s) by check or money order payable to U.S. Department of State and write your full name and birth date on the memo line:
