“Let’s Talk Story,” an interactive program with Kōkua Mau, Hawai‘i’s Hospice and Palliative Care Organization, will help people start thinking and talking about how they make decisions, who should be involved, and what types of medical care they want and don’t want. It will also cover basics of advance care planning and an overview of the Advance Health Care Directive (AHCD) and POLST (Providers Orders for Life Sustaining Treatment).

Kōkua Mau is Hawai‘i’s ‘Movement to Improve Care’ and foster conversations. We are a network whose collaborative membership includes health plans, hospice providers, hospitals, palliative care experts, long-term care providers, educational institutions and passionate community champions.
